Thank you Doctor for your very insightful questions!
Let me try to answer them !

1) definitely you should recheck your vitamin D level at this point. Depending on your way to 2000 IU may not be enough. The only way to know what is to follow it sequentially at several month intervals to get a feel for how your biochemistry works with vitamin D. Please be sure you are using a reliable reputable type of vitamin D or all bets are off.

2) I have really not seen anyone who did not respond to two months of high-dose vitamin D. The levels always go up.

3) I'm not familiar with the connection between rib cage muscle spasms and vitamin D deficiency. Have you checked your red blood cell magnesium level? I suspect you may be low in magnesium. Certainly with the vitamin D to help it work you should be taking a calcium — magnesium supplement.

4) I'm not aware of any association between low vitamin D and bile malabsorption problems.

I just finished your book - fantastic! I was diagnosed with deficient levels of Vitamin D & after 8 weeks of 50,000 D2 per week began D3 1000iu each evening. I also have idiopathic bile malabsorption & am taking cholestyramine (each morning).

Over the past 8 years, I've experienced severe ribcage pain which typically occurs in the evening (as painful as my 4 natural childbirth experiences) & is worse with cold, in the winter & with use of my upper extremities (like holding a baby or carrying a large pot of water). I can manage the symptoms with Flexeril 5mg qhs prn, but as this causes sedation, would prefer to handle this naturally. After reading your book, I realize that I am very overdue in rechecking my 25-OH D level (and I'll increase to 2000iu if my level isn't around 50). As an MD, I've tried to research a connection between ribcage muscle spasms & Vit. D deficiency, but haven't found anything. Have you heard of this in others who are D-deficient? Have you ever seen rickets of the ribcage? Also, in the book, response to D repletion seems to be fairly rapid. Realizing now that my lack of improvement may be due to inadequate repletion, I wonder if you've seen improvement take months/years. Here's another random question: Have you heard of any association between low Vit. D & bile malabsorption/intestinal problems other than IBD? Thank you for producing such a tremendous resource & for any guidance you can give! Have you considered producing a printable resource that MD's can give to their patients referencing your book & website?
