Thank you Doctor for your very insightful questions!
Let me try to answer them !
1) definitely you should recheck your vitamin D level at this point. Depending on your way to 2000 IU may not be enough. The only way to know what is to follow it sequentially at several month intervals to get a feel for how your biochemistry works with vitamin D. Please be sure you are using a reliable reputable type of vitamin D or all bets are off.
2) I have really not seen anyone who did not respond to two months of high-dose vitamin D. The levels always go up.
3) I'm not familiar with the connection between rib cage muscle spasms and vitamin D deficiency. Have you checked your red blood cell magnesium level? I suspect you may be low in magnesium. Certainly with the vitamin D to help it work you should be taking a calcium — magnesium supplement.
4) I'm not aware of any association between low vitamin D and bile malabsorption problems.
5) thank you for the idea of creating a printable resource that MDs can give to their patients. I have produced such a resource and it comes as a free gift for those who buy my new DVD which reviews my book. If you would please send me your e-mail [email protected], I will be happy to send you a complimentary copy of this PDF file.
With best wishes,
Dr. Soram
